Synthetic cannabinoids are potent agonists of the cannabinoid receptor type 1 (CB1) and type 2 (CB2).

CB1 receptors are distributed in the central and peripheral nervous system, whereas CB2 receptors are distributed outside the nervous system.

Synthetic cannabinoids bind to these cannabinoid receptors stronger than delta9-tetrahydrocannabinol (THC) [9],

the primary psychoactive alkaloid in Cannabis sativa, the plant commonly known as “marijuana” or “hemp,” which has been used as a source of fiber, oil, and medicine and for recreational and religious reasons since antiquity.

Their metabolites also serve as agonists for the CB1 and CB2 receptors. These characteristics account for the stronger potency of synthetic cannabinoids, which make the recreational use of these compounds more dangerous, possibly leading to fatal consequences.

The recreational use of synthetic cannabinoids was first noted in Europe in 2004 and then in the US.

Because of the drastic increase in synthetic cannabinoid-related cases, these compounds were identified as an emerging public health threat in the 2010 annual report of the American Association of Poison Control Centers’ National Poison
